Creating the Initial Form

  1. Browse to the page containing the Form Wizard Pro Module (or add this module to an existing page)
  2. Click the "Edit Form" link at the top of the module
  3. Press the "Add New Question" button
  4. Type your question in the "Question Text" box
  5. If the user must answer the question, check the "Answer Required" checkbox
  6. Select the type of answer field most appropriate for your question from the "Question Type" drop down box
    • Text - A short line of text, good for names and email addresses
    • Paragraph Text - A box for entering text, good for descriptions and open-ended questions
    • Choose From a List - A dropdown list, where the user can select one of the options
    • Checkboxes - A list of items, where the user can check as many items as they like
    • Multiple Choice - A list of items, where the user can check one radio button for their preferred option
    • Date Picker - A small calendar, where the user can choose a single date
    • Sliding Scale - A slider bar that goes between one and ten
  7. Press the "Save" button below the "Question Type" box to save your changes
  8. Press the "Finished" button above the question to return to the form editor
  9. Repeat steps 3 - 8 until you have added all your questions
  10. Press the "Save" button at the bottom of the form editor to save your form
    • You may need to click the menu link to your form page after you have pressed "Save" in order to view your changes.
