Creating the Initial Form
- Browse to the page containing the Form Wizard Pro Module (or add this module to an existing page)
- Click the "Edit Form" link at the top of the module
- Press the "Add New Question" button
- Type your question in the "Question Text" box
- If the user must answer the question, check the "Answer Required" checkbox
- Select the type of answer field most appropriate for your question from the "Question Type" drop down box
- Text - A short line of text, good for names and email addresses
- Paragraph Text - A box for entering text, good for descriptions and open-ended questions
- Choose From a List - A dropdown list, where the user can select one of the options
- Checkboxes - A list of items, where the user can check as many items as they like
- Multiple Choice - A list of items, where the user can check one radio button for their preferred option
- Date Picker - A small calendar, where the user can choose a single date
- Sliding Scale - A slider bar that goes between one and ten
- Press the "Save" button below the "Question Type" box to save your changes
- Press the "Finished" button above the question to return to the form editor
- Repeat steps 3 - 8 until you have added all your questions
- Press the "Save" button at the bottom of the form editor to save your form
- You may need to click the menu link to your form page after you have pressed "Save" in order to view your changes.
